The thing is that I don't remember her ever citing back pain as a reason for wanting the reduction. I've only ever heard her mention her image and the way she is perceived. Ironically enough her getting tattoos was one of the worst things she could do in terms of being recognizable. Like just dye your hair and change up the style and you could have had plausible deniability.
